Location Energize		Complete this form anytime a location is energized for any reason other than outage restoration.

Location De-Energize		Complete this form anytime a location is left de-energized.

Meter Exchange		Complete this form anytime a meter is exchanged.

Transformer Exchange /Install/ Remove		Complete this form anytime one or more transformers are exchanged, installed, or removed. This should be filled out for Auto Transformers as well. Make sure to take pictures of nameplates.

Material Charge Ticket		Complete this form anytime inventory material is used without a work order.

Employee Self Observation Form		Complete this form monthly to track areas of focus for new safety training.

First Aid Incident Form		Use this form when there is an incident involving first aid or a hospital visit.

Near Miss Form		Use this form to enter any near miss that you are a part of, or a near miss that you witness.

Oil Spill Notification Form		Use this form if you discover our equipment leaking oil.

Property Damage Form		Use this form when there is damage to MWEC or publicly owned property.

URD Damage Form		Use this form when there is damage to any MWEC underground facilities.

Vehicle Incident Form		Use this form when there is an automobile accident.

Vehicle Inspection Form		Use this form to complete quarterly vehicle inspections.
